Tuesday and Thursdays 9:00 am to 3:00 pm or by appointment call Melissa White 318-238-0707 to complete the Pre-K Pre-Application go to our online portal at
https://apply.teachingtomorrownow.com/ and upload the document(s) listed below

● Copy of current and updated child’s immunization record
● Copy of child’s birth certificate
● Copy of child’s Social Security card
● License/ Photo I.D.
● Current Proof of Residence (current utility bill, mortgage, lease, Affidavit of
residency )
Proof of Income (need one of the following):
Two (2) CURRENT & CONSECUTIVE check stubs for EACH parent/guardian in the Household
An official letter from your employer
A statement from the Social Security Administration, if applicable.
If Zero income, submit a Statement of No Income,
Declaration of Irregular Income

If Applicable:
❏ Copy of Court orders/ legal documents
❏ Proof of Disability
